Dancers train intensively, repetitively, and continuously throughout their careers.<br \/>\nThe hips are subjected to extreme ranges of motion and repetitive high loads. <strong>Pain and injuries<\/strong> are therefore common throughout a professional dancer\u2019s life.<\/p>\n<p>A frequent concern arises : \u201c <i> What if I am a professional dancer under 40 with hip osteoarthritis? Can I return to professional ballet after receiving a total hip replacement before the age of 40? <\/i> \u201d<\/p>\n<p>These are questions our team regularly hears from dancers during consultations.<\/p>\n<h2>Signs and diagnosis of hip osteoarthritis in dancers<\/h2>\n<p>The onset of <a href=\"https:\/\/www.chirurgiedelahanche.com\/en\/pathologies\/coxarthrosis\/\"><strong>coxarthrosis<\/strong><\/a><strong> (hip osteoarthritis)<\/strong> in a professional ballet dancer manifests as <strong>mechanical groin pain<\/strong>, later radiating around the hip and down the thigh. A <strong>progressive loss of joint mobility<\/strong> develops, making dance movements increasingly difficult.<\/p>\n<p>Standard imaging typically shows <strong>joint space narrowing, subchondral cysts, and osteophytes<\/strong>.<br \/>\nSometimes, even before osteoarthritis appears radiographically, <strong>cartilage and labral lesions<\/strong> of the acetabulum can be detected. It is not uncommon to also observe <a href=\"https:\/\/www.chirurgiedelahanche.com\/en\/pathologies\/dysplasia\/\"><strong>hip dysplasia<\/strong><\/a> or <strong>femoroacetabular impingement (FAI)<\/strong>.<\/p>\n<p>When there is severe cartilage damage (with or without dysplasia, with or without osteoarthritis) <strong>arthroscopic or conservative surgery is no longer appropriate<\/strong>. At that stage, <strong>total hip replacement becomes the only viable option.<\/strong><\/p>\n<h2>Preoperative evaluation : a crucial step<\/h2>\n<p>For a professional ballet dancer, <strong>a comprehensive physical examination<\/strong> is essential, assessing leg length, joint range of motion, especially <strong>external rotation, turnout, flexion, and extension<\/strong>.<\/p>\n<p>While standard X-rays confirm the diagnosis, <strong>3D CT-based preoperative planning<\/strong> is indispensable for detailed analysis of hip anatomy and geometry.<\/p>\n<h2>3D Planning and custom-made hip prosthesis<\/h2>\n<p>Once the decision for hip replacement is made, the procedure is planned <strong>in 3D and life-size<\/strong>, using the patient\u2019s CT scan. A <a href=\"https:\/\/www.chirurgiedelahanche.com\/en\/treatments\/custom-made-hip-prosthesis\/\"><strong>custom-made prosthesis<\/strong><\/a> is then designed and manufactured specifically for the patient before being implanted by our surgical team.<\/p>\n<p>This 3D planning combined with a custom implant allows <strong>precise restoration of the native hip anatomy<\/strong>, optimizing:<\/p>\n<ul>\n<li>The <strong>center of rotation<\/strong><\/li>\n<li>The <strong>articular length<\/strong><\/li>\n<li>The <strong>medio-lateral offset<\/strong><\/li>\n<li>The <strong>femoral anteversion<\/strong><\/li>\n<\/ul>\n<p>Such anatomic precision restores <strong>muscular balance<\/strong>, <strong>joint mobility<\/strong>, and especially <strong>external rotation (turnout)<\/strong>, essential for ballet performance.<\/p>\n<h2>Choice of implants and surgical approach<\/h2>\n<p>Implant selection favors a <strong>cementless prosthesis<\/strong>, ensuring solid and durable bone fixation. A <strong>ceramic-on-ceramic bearing<\/strong> is particularly suitable for young, active dancers.\u00a0 In specific cases, a <strong>dual mobility cup<\/strong> may be considered.<\/p>\n<p>Surgery must always be performed through an <strong>anterior approach<\/strong>, which <strong>preserves the musculature<\/strong>.<br \/>\nIt is crucial to protect the <strong>external rotators, abductors, and periarticular muscles<\/strong> to maintain the dancer\u2019s balance and strength.<br \/>\nThe <strong>precise positioning of implants<\/strong> is key to reproducing the planned anatomy and ensuring optimal biomechanics.<\/p>\n<h2>Rehabilitation and return to dance<\/h2>\n<p>Postoperative rehabilitation is structured in phases, beginning with <strong>static muscle maintenance exercises<\/strong>, followed by <strong>gradual recovery of range of motion<\/strong>.<br \/>\nDance movements are reintroduced early, starting with <strong>floor barre work<\/strong> and <strong>physiotherapist-supervised exercises<\/strong>.<\/p>\n<p>The <strong>reconditioning phase<\/strong> is essential to achieve symmetrical recovery of all muscle groups, preventing future pain or injury upon return to dance.<br \/>\nA <strong>progressive return to professional-level ballet<\/strong> can generally be considered <strong>between three and six months<\/strong>, depending on postoperative recovery and preoperative joint condition.<\/p>\n<h2>Why resurfacing surgery should be avoided in dancers<\/h2>\n<p>In our experience, <strong>hip resurfacing is not recommended for professional ballet dancers<\/strong>.<br \/>\nThis technique has shown <strong>disappointing results in women<\/strong>, requires a <strong>posterior surgical approach<\/strong> that involves cutting the external rotator muscles \u2014 which is contraindicated \u2014 and uses a <strong>large-diameter cup<\/strong> unsuitable for dysplastic hips.<\/p>\n<h2>Excellent outcomes in professional dancers<\/h2>\n<p>In our published studies on <strong>professional ballet dancers under 40<\/strong>, outcomes are <strong>exceptional<\/strong>, with <strong>satisfaction scores between 9\/10 and 10\/10<\/strong> and <strong>over 95% returning to professional-level performance<\/strong> after surgery.<\/p>\n<p>These results confirm that, with meticulous <strong>3D planning<\/strong> , <strong>custom implants<\/strong>, and a <strong>muscle-sparing anterior approach<\/strong> , <strong>professional ballet can indeed be resumed safely and successfully<\/strong> after hip replacement \u2014 even before the age of 40.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Ballet dancers know that the hip joint is particularly vulnerable, and that total hip replacement is sometimes necessary after a professional career.
